A Medical visa is given to those who are travelling to India for a medical treatment. The visa can be single/multiple entry. Please note that relevant letters/documents explaining details of the treatment, signed and stamped by the concerned hospitals/doctors are required to validate the purpose of travel.The validity of the visa begins from the date of issue by the High Commission of India and not from the date of travel on your application form.The duration of the visa is at the sole discretion of the Issuing Authority. All visa fees once paid are non-refundable irrespective of the outcome of the visa application and duration granted. All the fields of the visa application form should be completely filled before submitting.
FRO offices forward the application to the SB Hqrs. Application of extension of Visa are forwarded by SP, SB(EZ), Assam, Kahilipara, Guwahati along with his recommendation and all necessary documents e.g. Passport, Visa etc to the state government. Extension may be accorded by state government as per Visa Manual and validity of the Passport.
International Travellers should have return ticket or onward journey ticket,with sufficient money to spend during his/her stay in India

Accurate and Complete Information
Provide accurate and complete information in all sections of the application form. Any discrepancies or incomplete information might lead to delays in processing your visa application.
Fill in your personal details, including your full name, date of birth, nationality, passport number, and contact information. Double-check the accuracy of these details before submission.
Purpose of Travel
Clearly state that you are seeking medical treatment at Sunway Medical Centre. Provide details about your medical condition, the expected treatment or procedure, and the name of the treating doctor or specialist.
Duration of Stay
Estimate the duration of your stay in Malaysia for medical treatment. Ensure that your estimated duration aligns with the recommended treatment period provided by the medical professionals at Sunway Medical Centre.
Proof of Medical Appointment
Attach a copy of your appointment confirmation or a letter from Sunway Medical Centre verifying your scheduled medical consultation, treatment, or surgery.
Provide evidence of sufficient funds to cover your medical expenses, travel expenses, and stay in Malaysia. This could include bank statements, letters of sponsorship, or evidence of insurance coverage.
Supporting Medical Documents
Include copies of medical reports, diagnoses, and treatment recommendations provided by your medical professionals. These documents help the authorities understand the necessity of your medical travel.
Outline your travel itinerary, including flight details and accommodation arrangements. Ensure that your travel dates coincide with your medical appointments and treatment schedule.
Ensure your passport is valid for at least six months beyond your planned departure from Malaysia. Attach a clear photocopy of the passport's bio-data page with your application.
Attach two recent passport-sized color photographs with a white background. Follow the specified dimensions and guidelines for photographs.
Declaration and Signature
Sign the application form and any declarations accurately and as required. In case the application is for a minor or dependent, the parent or guardian should sign on their behalf.
